Ini merupakan Borang Tempahan Esports Akademi Pulau Pinang yang perlu diisikan untuk tempahan penggunaan bagi sesi pengenalan modul "Game-On: Esports The Right Way" yang dibuka kepada semua sekolah di negeri Pulau Pinang.
Tempahan yang dibuat akan diproses dan dimaklumkan semula sekiranya diluluskan berdasarkan kelapangan tarikh tempahan tersebut. Borang ini perlu diisi oleh cikgu/guru yang bertanggungjawab.

This is the Penang Esports Academy Reservation Form that needs to be filled in to reserve the use of the "Game-On: Esports The Right Way" module introduction session which is open to all schools in the state of Penang.
Reservations made will be processed and notified again if it is approved based on the availability of the reservation date. This reservation form must be filled in by the teacher in charge.
