Thank you for supporting your fellow students and taking part in Condom Connection! Complete the following form to become a sexual wellness resource to your students by making safer sex products available.

Remember, you have 1 refill of condoms each semester. Visit healthpromotion.msu.edu/sexual-wellness to fill your refill form when your out of supply. Don't forget that non-latex condoms, dental dams, internal condoms & lubricant are also available upon request.

The information we collect will remain private. Address information is requested for on-campus refill delivery through campus mail during this pandemic.
