This is an ‘Application to Train’ google form.

This form must be completed by all divers, new and returning, who wish to dive and those taking a break in Term 2.

The form begins with ‘Are you training this term?’

If 'TAKING A BREAK', the form will submit and end. Thank you for informing us.

If 'Training' your request will be processed and an invoice will be generated depending on your responses to the number of training sessions selected. Athletes can not train unless this form is completed.

Club GSV (PLC students Year 7-12 ) are also to complete this form and are considered Club Members.

Club Membership Fees are included in term fees. Please continue for squad information and training dates.
