Olivet Kennel Daycare & Boarding Evaluation Questionnaire
We appreciate you considering Olivet Kennel where we treat your 4-legged friend like family. Please take a few minutes to answer the following questions. Upon receiving your completed questionnaire we will be in contact within 72 business hours. Our evaluation includes daycare and overnight boarding ($65-70 non-refundable fee) after which you will receive a full report on your dogs visit, suggestions on how we will proceed with future bookings, plus access to our online customer portal. DHLPP (Distemper booster), Bordetella (Canine cough), Rabies and Canine Influenza (
