This form should only be filled out if you require shipping, require an invoice ahead of time, are tax-exempt, need more than 20 gallons, or have some other special request.  If none of these apply to you, you may purchase our hand sanitizer liquid at our front door during regular business hours.  Mon-Thurs 12-10 and Fri-Sat 12-11.  We will have a large amount of product available on June 1st and should be able to fill any size order immediately.
Our price is $130 for a 5 gallon bucket.  Price breaks for very large orders are available.  Our product is labeled and contains 70% ethyl alcohol with glycerin and hydrogen peroxide.  We accept payments as cash, business check, or credit card.  Orders of 10 gallons or more will be charged the processing fee for credit cards payments.
