I understand that the open gym held at ACC Youth Center (62 W. 7 Mile Rd. Detroit, MI 48203) may put me at risk for accidents, injury, illness, or death. I understand that there are risks inherent in any physical activity, including but not limited to falls and injuries sustained as a result of contact with other participants. I assume all responsibility for such risks. I understand it is my responsibility to communicate any physical or psychological concerns that may prevent me from taking part in an activity. I confirm I am physically and mentally able to perform every chosen activity. As I have read this waiver and acknowledge these facts, I agree to waive any responsibility or liability from Bethel Baptist Church East, its board, its officers, employees, agents, representatives, organizers. This includes anyone entitled to act on my behalf arising out of my involvement in the open gym.
