Please use this form to request access to an iPad (Grades K-2) or Chromebook (Grades 3-5) in the event of a prolonged school closure due to Coronavirus Disease 2019 (COVID-19).  If you have more than one child needing a device, please fill out one form per child. You can pick up your child’s iPad\Chromebook from their school during the following times:

Wednesday March 18th from 7:30 AM to 9:00 AM or 2:00 PM to 4:00 PM
Monday March 23rd from 7:30 AM to 9:00 AM
