This form is for all employees to use to report an illness and/or quarantine due to COVID-19 Pandemic. All back-up documentation must be submitted directly to Thomas Stack, Assistant Superintendent for Personnel and Pupil Services by email. All documentation must be complete in order to continue to receive pay.
