Madden Elementary teachers are eligible for reimbursement, up to $50.00, from Madden Elementary PTO for items personally purchased for classroom use. Please review the guidelines below and complete this form in full to receive your reimbursement.
• Sales Tax will not be reimbursed
• Receipts must be submitted via EMAIL to
madden.treasurer@gmail.com. Subject: Classroom Enhancement Reimbursement Request
• Receipts must be submitted within 60 days of purchase, with a final deadline of May 1.
• Teachers are only eligible for one reimbursement check per school year. Please consolidate your receipts until you’ve reached $50 maximum and submit only one form.
• Teachers must be current on PTO Membership dues to participate.
• Reimbursement forms will be processed monthly.
• Reimbursement checks will be placed in teacher mailboxes.
