If you did not order a yearbook, but would like to purchase one, we have a limited number of yearbooks for sale for $15. Please complete this form prior to Friday, June 5 and we will place the yearbook in your child's bag. When you come to pick up the belongings, please place exact cash or a check, made out to Deerfield Elementary School, in an envelope labeled yearbook in the trunk. We will not be able to provide any change.

There are a limited number of yearbooks for sale. Filling out this form does not guarantee that your child will get a yearbook. If you fill out this form and we are out of yearbooks, we will email you to let you know.
