In consideration of being permitted to use Revive Fitness gym and equipment, on behalf of myself, my family, and my assigns, I hereby release Ana Betancourt, Revive Fitness, its owners and operators from liability for injury, death , or property loss suffered by the resulting from the ordinary negligence of Revive Fitness, its owners and operators while I'm using the facility, equipment or in any way associated with participating in any and all gym activities now or in the future.
I acknowledge that I know, understand the inherent risks of using Revive Fitness, equipment and participation in the group training sessions, activities and program instructions, including without limitation use of the equipment, or facility. I know and understand that these risks can range from minor scrapes, strains, and bruises to significant injuries such a s broken bones, eye injury or concussion , paralysis and even death. By execution of this agreement , I fully assume the inherit risks associated with an athletic club use and assert that I am voluntarily participating in such activities.
I understand commitment and attendance is necessary to reach my goals.
