In consideration for being permitted by Mary Rose, Nathaniel Holder, Yocto Theatre, and Portland Action Theater to participate in their "Action Theater Workshops"  commencing on or after the date below, I, for myself, my personal representatives, heirs and assigns hereby:

Acknowledge, agree and represent I understand the nature of “Action Theater Workshop” activities and declare I am qualified, in good health and proper physical condition to participate in  “Action Theater Workshop” activities;

Warrant that if at any time I determine any conditions to be unsafe, I will immediately discontinue further participation in the activity(ies);

Fully understand the risks and dangers inherent in my participation, my own actions or inaction as well as those of other participants; and as such

Waive, release, covenant not to sue, and discharge any and all claims for damages for personal injury, death, or property damage that I may have, or which may hereafter accrue to me, as a result of participation in said  "Action Theater Workshop” activity.

This release is intended to discharge in advance Mary Rose, Nathaniel Holder, Yocto Theatre, and Portland Action Theater, its instructors, officers, employees, agents, sponsors, advertisers, and, if applicable, owner and/or lessor of premises (hereinafter referred to as “Yocto”) on which "Action Theater Worskhop” takes place, from any and all liability, claims, demands, losses or damages arising out of or connected in any way with my participation in said activity, even though that liability may arise out of negligence or carelessness on the part of the persons or entities mentioned above. It is further agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assigns. I agree to indemnify and to hold Yocto harmless from any loss, liability, damage, cost, or expense which they may incur as the result of my death or injury or property damage that I may sustain while participating in said activity.
