門票登記須知 Notice to Applicant
1. 此表格用作登記預留本節目門票。活動費用全免，門票數量有限，每人限取 2 張。先到先得，額滿即止。每個電郵地址只限登記一次。
This form serves for the registration of admission ticket of the captioned programme. Free tickets are distributed on a first-come-first-served basis. Each patron may get a maximum of 2 tickets. Each eligible e-mail account can only be registered once.
2. 成功登記人士將會收到確認電郵。如有疑問，請於辦公時間內致電 6345-8473 查詢。
Confirmation e-mail will be sent upon successful registration. For enquiries, please contact 6345-8473 during office hours.
3. 主辦及製作單位有權拍攝及錄影活動及公開演出情況，作日後宣傳及紀錄之用。
The presenter and production unit reserve the right to capture and record the event and public performance. The photos and videos taken will be used for future publicity and documentation purposes.
4. 主辦單位保留更改節目內容、日期、地點、參與演出的學員及表演者的最終決定權。
Programmes are subject to change without prior notice.
5. 表格上的個人資料均由申請人自願提供。除個人資料(私隱)條例所訂明的豁免外，根據同一條例第18及22條及附表1第6項原則，申請人有權查閱及修改本表格所提供的個人資料。申請人若對本表格所收集的個人資料有任何問題請電郵info@sdz.com.hk與SDZ聯絡。
The provision of personal data by means of this form is voluntary. Subject to exemptions under the Personal Data (Privacy) Ordinance, and in accordance with Section 18 and 22 and Principle 6 of Schedule 1 to the Ordinance, you have a right of access to and correction of the personal data provided in this form.
