MTK Registration Waiver

This agreement releases the Mississippi Thunderkings Hockey Club all of its Directors, members, non-members, players, volunteers and affiliated associations from all liability relating to injuries that may occur in any and all activities associated with the MTK Hockey Club. By acknowledging this agreement below, I agree to hold the MTK hockey Club entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in these activities and I swear that I am participating voluntarily. By acknowledging below I forfeit all right to bring a suit against the Mississippi Thunderkings for any reason.
