Please fill in this form to indicate your child's afterschool transportation each day. The first 10 days of school we ask that there be no ride changes so that students, teachers, bus drivers and staff members can get into a rountine.
After the first 10 days, students will need a note from home that includes: parent signature, child's first and last name, teacher's name, date and how the child will be going home (if someone else or a bus, indicate the name and/or bus number).
If you need to make a change after the school day begins, please call the main office before 11:00.

Afternoon transportation options:
CAR RIDER: Please have your orange car rider ID (family number) on your dashboard. Remain in your car.
WALKER: Only students who live within ½ mile of school and are not eligible for a bus are allowed to be walkers. These students exit at the “old entrance” facing Doncaster Road.
BUS RIDER: K and 1st grade bus riders will not be allowed to leave the bus without an escort or 4th and 5th grade sibling. If no adult/escort is available at the bus stop, the child will be taken to the next school on the route and parents will be contacted to pick up from that school. Bus riders must have signed Attestation form on file at Sherwood.
