HVPAA Care Transformation Tournament
Medical centers are invited to participate in this competition by submitting your very best value-based care transformation project. HVPAA directors will vote to determine the best initiatives, and those institutions that advance to a final 4 play-off session at the 2020 conference (www.hvpaa.org/2020nc). Winners take home this beautiful bronze statue of Hippocrates.

Institutions may submit more than 1 care transformation initiative. Published and previously presented projects are welcome. All submissions due June 1st.
