Police Complaint/Commendation Form
The purpose of this form is to allow civilians to voice their concerns or provide positive feedback about an individual employed by the San Andreas Department of Public Safety. This form should not be used to inappropriately or falsely accuse employees of the SADPS.

Information provided using this form will be completely anonymous to the individual you are reporting. All information is sent directly to the San Andreas State Police. They are the only agency that will know the details of this report.

* Reports on a Trooper of the San Andreas State Police may be forwarded directly to the Director of the State Police.

When utilizing this form, you must be truthful and provide evidence (in the case of a complaint) to justify your report. Complaints that do not provide evidence, are not completed fully or are determined to be dishonest will be dismissed.
