IN-PERSON Private Lesson Request Form
Our IN-PERSON private lessons are perfect for up to 4 family members in your "cohort". Once you complete this form, you will be matched with an appropriate coach.  Your assigned coach will contact you to gather more information on what you are looking to learn and set up a time that works for both you and the coach to meet at Phoenix Gymnastics. You will then be sent a link to register and pay online through your Amilia account.  You can track your payments and lessons through Amilia.  If you are looking to book again you can either contact your assigned coach or you can fill out a new request form.
