As a parent or guardian, I hereby give permission for the child that I registered to participate in the Vacation Bible School (VBS) at Edmonds Lane Bible Chapel in Lewisville, Texas. This includes all sponsored activities at the church property. I understand that reasonable precautions will be exercised by the adults chaperoning the event. This permission shall remain in effect until and unless terminated in writing. I also hereby give permission for child to ride in any vehicle transporting the children to and from the VBS. I understand that drivers must be over age 21 and approved by Edmonds Lane Bible Chapel. Parents and participants should understand that participation at the VBS is not a right but a   privilege   that   can   be   revoked   if   the child behave inappropriately or if they pose a risk to themselves or others. I further understand that my signature authorizes emergency medical treatment to be administered, if necessary to the child. I hereby release Edmonds Lane Bible Chapel, its   agents,   adult   volunteers   and   drivers from responsibility and liability for any injury or illness or damages that my child or I may sustain as a result of the child attending the VBS.
