BLS Textbook Pickup for IEMS CPR Class
If you have registered for a BLS class through IEMS, please fill out this form to arrange a time to pick up your book. The American Heart Association requires that all students pick up the textbook 2 weeks before this class. We will not allow any exceptions to this rule, and a textbook is required for entry to the class.

The CPR Education Department staffs Office Hours on Tuesdays between 4pm and 6pm at the Illini Union. Would you be available to pick up your textbook within that time? *
The IEMS office is located in the Illini Union (1401 W. Green Street, Urbana, IL 61801). Within the Illini Union, our office is located on the Mezzanine Level, which can be accessed through the “Catering” door located next to the Credit Union (by the East side of the building).
If you are unavailable to come to our Tuesday office hours, please email your availability to Bhavya Joshi (bjjoshi2@illinois.edu) our BLS Class Coordinator.
I have reviewed the cancellation policy for the course and understand that IEMS reserves the right to charge for the class, in full, if not notified of a cancellation request at least 24 hours before the course. In addition, I understand that I must pick up my textbook 2 weeks before the course. Students who cancel at least 24 hours before the course begins are required to return their textbook (in like-new condition) to the IEMS office within one (1) week of the course to avoid being billed the full value of the book ($16). *
