Post- Completion OPT Reporting Form
Students on post-completion OPT are required to report all employment within 10 days of the start date of the job.  You can do this one of two ways:

SEVP PORTAL

On the date your OPT authorization becomes active, you will receive an email
from this email address: do-not-reply.SEVP@ice.dhs.gov,
with the subject line, "Optional Practical Training Approval - the next step".

This email will be sent to the email address you submitted when you applied for OPT.  You should follow the instructions in this email within 14 days of the time you receive it to set up your account.

After you have access to the SEVP Portal, you can update your contact information and your employment information via that Portal. If you are adding new employment more than 10 days after your start date, the system will not allow you to update your information yourself.  In that situation you should use this form.

REPORTING FORM

Prior to the start date of your OPT, you may submit future employment via this form. You may also use this form at any time during your OPT to report employment information that you could not or choose not to update via the portal.
