To complete registration for any of the 2021 Multi-Arts Summer workshops you must:
•Complete this online registration and submit
•For new multi-arts' families: Sign and return Release of Liability Agreement (to multiarts.org@gmail.com)
•Send certification of up-to-date immunizations/most recent physical exams (to multiarts.org@gmail.com)

I give the Director and staff of Multi-Arts permission to administer minor first aid to my child. Should more than a minor injury occur, requiring emergency treatment, I give permission to the Director and staff to acquire emergency medical treatment (i.e. ambulance).

Photograph and video release (No profit will be made from these recordings, and the names of the children will not appear next to the images) *
