Parent Survey 2021-2022
Dear Parents,
The year has now begun and it is my hope that it will be a successful one for your child.  For a teacher, knowledge of your students can be a powerful asset in making sure they have success.  Your knowledge about your child is one of the most valuable resources I have.  For that reason, I would ask that you take some time and fill out the survey below in order to help me make this a strong year for your son or daughter.  Even if you cannot answer all of the questions, please fill out what you can and send it back to school.  If you are uncomfortable answering any of these questions, feel free to skip them but any information would be valuable.  If you can, please complete it by next Friday, September 4.  I will take late returns of this in case you need more time.

Academics
Overall, what do you feel is/are your child’s greatest academic strength(s)?  This doesn’t necessarily mean a subject area. It could be a specific topic or work habit such as multiplication or organization.  If you do state a subject area such as math or reading, any specifics you could give would be helpful.
What, if any, do you feel is your child’s greatest academic challenge(s)?  Similar to the above, it could be specific topics or work habits.
How long does it take on average for your child to finish their homework?  assume a night of homework tends to be 2 and sometimes 3 assignments to be turned in and they will not necessarily have it every night.  Example: 1 math, 1 language related (reading, writing, spelling) and possible 1 science or social studies.
Social
What are your child’s interests?  What activities do they enjoy participating in or topics they seem most interested in?  What are some things they take part in outside of school hours?
What days or times of the year are they involved in those?
Would you describe your child as more of an introvert or an extrovert? Feel free to elaborate.
Does your child’s social circle tend to be with a small group of one or two good friends or do they tend to be more prone to play/hang out with several different kids?
Does your child do chores or have any responsibilities around the home?  If so, what kind of chores and how often do they do them?
Technology Access
We use a lot of technology in our classroom for instruction and assessment and we are always finding new ways to use it.  Below are some of the programs and apps we have used with the students and parents in the past or may use in the future.  Please mark which ones your child or you have used before on your own or with teachers in the past.  If you aren’t sure if your child has used the program, you can just leave it blank.
 The above apps and programs are all web-based and are used primarily at school.  But students are encouraged to use them at home.  All of these are usable with a computer and most can be used with a tablet or smartphone.  All of them require access to the internet.  Please check which devices your child would be able to use at home to potentially access these sites.  (Mark all that would apply)
