DIRECTIONS:  Please submit only ONE form for changes to your schedule.  Schedule changes will be processed by your alpha- group counselor in the order they are received.  Students making schedule change requests will ATTEND THEIR ORIGINAL ASSIGNED SCHEDULE UNTIL THE CHANGE FORM IS PROCESSED and approved, you may attend your new schedule immediately and it will be visible in your StudentVUE account. The student will be responsible for collecting and completing any missed assignments from the new course if the change is made after the course has started.  Course requests will be honored when possible; however, classes and teachers will NOT be overloaded to fulfill requests.  **Please check the Master Schedule on the CHS Counseling Home Page to see what class is offered during what period for conflicts and see the Program of Studies on the Home Page to see what prerequisites are required.
