Transportation Plan Form 2023-24
In years past, we required a form for all students.  This year we are only requiring students who need transportation to be provided to and/or from school to have a Transportation Plan Form for 2023-24 School Year on file in the Transportation Office.  (Fill out a form for each student that needs transportation)

If you are attending UVCC, your form will need to be turned in by August 1st to ensure your child has a bus prior to the start date of August 17th.

**ONLY**If your child needs transportation to AND/OR from a 2nd court-ordered shared parenting address you will need to fill out our Shared Parenting Waiver and Calendar Forms. You will need to provide your Court-Ordered Shared Parenting paperwork for us to make a copy for our records.  ALL Forms are due by August 8th, 2023, to ensure we have the time to assign and send out communications to parents before the start of the school year.  Contact our office and we will send you the links for those forms OR you can come to the Transportation Office to fill them out.
