一、規章辦法
請詳閱「國立臺灣大學醫學圖書館校外人士文獻傳遞服務辦法」

二、申請流程：
(1) 初次申請者，請先至醫圖臨櫃填寫「預存基金表」並預存300~3000元
(2) 填寫本表單
(3) 等待Email或電話通知取件（郵寄or醫圖櫃台）
(4) 以Email提供本次申請收費明細表

若臺大醫圖無館藏，則不代為申請國內外館際合作服務，請見諒。
