I understand that to use the Student Association Lectern:
1. I am required to pay a deposit as documented at the top of this form.
2. I will forfeit the deposit if I do not give JCUSA 24 hours’ notice that the booking is cancelled.
3. I am aware that if I return the item late that JCUSA will retain my deposit.
4. The lectern must be clean before returning it to the Student Association. Failure to clean the equipment used and any damage to equipment will result in the deposit being forfeited and may incur additional penalties for repair of damage.
5. The user will be liable in the event that they damages the lectern unless otherwise negotiated with the Student Association President or the General Manager.
6. Any equipment borrowed must be securely stored under lock and key overnight.
7. JCUSA will not take any responsibility for personal injury or public liability regarding the use of the lectern.
8. Failure to abide by this agreement will result in the forfeiture of any future use of Student Association property.
9. I undertake to be bound by and comply with these requirements in every respect and I further undertake to be responsible for ensuring that all individuals or groups using the equipment in association with this application shall comply with the Safety Briefing requirements.
10. I will inspect the equipment and acknowledge it is clean, damage free and will be suitable for the intended use.
11. I indemnify JCUSA from any claim/s arising from any incident/s occurring with the use of the equipment during the utilisation period.
12. I acknowledge that I should wear enclosed shoes to protect my feet and not wear any loose clothing or jewellery that could get caught.
13. I have been trained in the use of the equipment.
