CTLE Credit Request Form
NYS-MEP Staff seeking Continuing Teacher Leader Education Credit (CTLE) for completing On Demand Professional Development modules accessed through nysmigrant.org, please follow this procedure:
 
1. Complete this CTLE Credit Request form; and
2. Alert the NYS-MEP Technical Assistance Support Center (TASC) and your METS Director with an email stating which PD module you completed. The email should be sent to Jennifer Verdugo, NYS-MEP TASC Special Projects Coordinator at jennifer.verdugo@oneonta.edu and C.c. your METS Director.
3. We will be in touch with you after receiving both the email and this CTLE Credit Request.

Note: If you are seeking credit for multiple modules, complete a new CTLE Credit Request for each.
