COVID-19 Screening Testing for Students

NYS Department of Health mandating all P-12 schools to offer screening testing for students once per week. Students DO NOT have to be tested, this is just a service we are offering if you are interested in having your child tested weekly.

Per Governor Hochul:
"COMMISSIONER’S DETERMINATION: COVID-19 Testing in Schools: Issued pursuant to 10 NYCRR 2.62.
-P-12 schools are required to 𝗼𝗳𝗳𝗲𝗿 screening testing to students once per week, with appropriate parent and guardian consent for minors..." 𝗡𝗼𝘁𝗲: 𝘀𝘁𝘂𝗱𝗲𝗻𝘁𝘀 𝗮𝗿𝗲 𝗻𝗼𝘁 𝗿𝗲𝗾𝘂𝗶𝗿𝗲𝗱 𝘁𝗼 𝗽𝗮𝗿𝘁𝗶𝗰𝗶𝗽𝗮𝘁𝗲 𝗶𝗻 𝘁𝗵𝗲 𝘀𝗰𝗿𝗲𝗲𝗻𝗶𝗻𝗴 𝘁𝗲𝘀𝘁𝗶𝗻𝗴 𝗽𝗿𝗼𝗴𝗿𝗮𝗺.

We are using this survey to inform our planning for the size and scope needed for the screening/testing program. Additional information will be shared once a plan is finalized.

Please complete this form by October 20th to opt your child into into the testing program. Testing will happen one time per week and is free of charge. Information from testing will be shared with Livingston County Department of Health.
