I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Bison Backcourt Club (referred to as the BBC) has put preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that the BBC can not guarantee that my child or myself will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others. I voluntarily allow my child to participate in BBC Traveling Tryouts and Traveling Basketball and acknowledge that I am increasing my and my child(ren) risk to exposure to the Coronavirus/COVID-19.

I acknowledge that my child(ren) and myself must comply with all set procedures to reduce the spread while attending tryouts. COVID-19 Screening BBC families are being asked to screen (series of questions) their child(ren) before sending them to Tryouts, practices, and tournaments.

Likewise, BBC staff and visitors in our schools must also participate in a daily COVID-19 self-screening. Below is a check-list of questions parents/guardians and staff should answer before entering:
My child is not exhibiting any of the following symptoms associated with COVID-19:

~New-onset cough~Shortness of breath
~fever (100.4 or higher)
~chills - muscle pain
~sore throat
~loss of sense of smell or taste
~Runny nose
~gastrointestinal symptoms of diarrhea, vomiting, or nausea

~My child has not been in close contact with anyone within the last 14 days who have been quarantined or isolated due to COVID-19. My child has not been advised by any medical provider or health authority that he/she may have been exposed to COVID-19.
~I have followed the Centers for Disease Control and/or local government's recommended travel guidelines for my student. I have quarantined for the required time upon return from travel.
~I will immediately notify the BBC if I cannot self-certify for any of the above reasons for my child(ren).
~ I also understand that in the case that I am notified that my child(ren) may have been exposed to COVID-19 OR start to exhibit any of the symptoms above while at Practice, Games or Tryouts, I will immediately pick-up my child(ren) when contacted by the BBC Board or Coaches. I understand that myself and the child(ren) must wear a face-covering over the nose and mouth at all times while in the building.
