Thank you for offering your services and talents to benefit the young people of one•n•ten! Please review and sign this Special Guest Code of Conduct form, acknowledging and agreeing to abide by the following standards. Should you need clarification, please contact one•n•ten staff at 602-400-2601.

1. I will make the safety and well-being of the one•n•ten youth the basis of my decisions and actions while at one•nten. *

2. I will abide by the safe space guidelines set forth by one•n•ten and respect the rules of the space with regards to general operations (i.e. set up, clean up, closing time, etc.). I will also respect the direction and requests of one•n•ten staff. *

3. I understand that for my safety and the safety of one•n•ten youth, I am never to be alone with a single youth where others cannot observe me. *

4. I understand and agree to treat all youth with respect, consideration and care, and treat all youth equally regardless of gender identity, sex, sexual orientation, age, ability, ethnicity, race, spirituality, class, or culture. *

5. I will portray positive role modeling for youth by maintaining an attitude of respect, loyalty, patience, and maturity. *

6. I understand that all contact with youth shall occur during official one•n•ten events or programs. Personal contact information (i.e. phone number, email address, home address, etc.) will not be shared with youth participants of one•n•ten. I understand that it is unacceptable to have a private or romantic relationship with one•n•ten youth. *

7. There may be times that you see one•n•ten youth or volunteers in public. Since youth may not be “out” please do not approach youth to say hello unless they say hello to you first. This is to protect the privacy and confidentiality of our youth. An exchange of hello is appropriate, but no further contact should occur. *

8. I understand that one•n•ten prohibits volunteers and guests from using tobacco, drugs, and alcohol at one•n•ten. *

9. If performing, I understand that all songs must be edited and will not contain profanity, will not be overtly sexual, and will not contain racist, homophobic, sexist, or otherwise bigoted or inappropriate language or themes. *

10. All performances must be catered for a “PG” audience. Performance props, dancing, and clothing may not be sexual or offensive. Clothes must cover all private areas, including stomach. All dressing/changing into and out of costumes must be done in private with no youth present. *

I have read and fully understand the one•n•ten Special Guest Code of Conduct and agree to abide by the above rules and guidelines. I understand that if the above rules and guidelines are not adhered to I will be asked to leave the one•n•ten property and will not be invited back into programs.
