To appeal a Level One decision, or the lack of a timely response after a Level One conference, please fill out this form completely and submit it by email to grievances@mvisd.org, fax 210-941-0456, in person at the MVISD Central Office located at 8449 FM 471 S., Castroville, TX, or by U.S. Mail to 8449 FM 471 S., Castroville, TX  78009, within the time established in FNG (Local). Appeals will be heard in accordance with FNG (Legal) and FNG (Local) or any exceptions outlined therein.
