Fit Life Challenge 2022 Application Form
The Fit Life Challenge Blends the Components Needed to Get Fit!

This is for anyone at any age and any fitness level. Enhancements for 2022 include options for MyPrimal Health Coaching & biometric data analysis with InsideTracker.

In previous challenges, some had signed up, but did not participate, so this application is to ensure that you are committed to taking part in the program.

You can choose to apply to participate in all, some or none of the components for the program. Please apply to all the ones you would like to be involved with.

The base costs are covered by DLP. You will have the option to upgrade to enhanced options, per below. Only 50 spots are available. Please complete the form below to apply to join the March - June Fit Life Challenge at DLP.

With the multiple parts to this year's challenge, for the next three questions, please indicate if you are applying to participate in this component: The Overall Fit Life Challenge Program (this includes Member access to Lessons, Videos, Guides, Checklists, Workout Programs, Bi-weekly Live accountability calls (most of which will take place Tuesday at noon EST & Recordings for those who cannot join live), Private Facebook Group (& on MyDLP), Nutrition Chart, Sample Meal Plans, Optional DLP Group activities, etc.)

Please indicate if you are applying to participate in this component: The MyPrimal Coaching Program (this includes working with certified health coaches, daily educational materials (text, audio, video) that guide a healthy life, in app text messaging with health coach, 6 live bi-weekly webinars with health coach, most of which will be Tuesday at noon & recordings available, progress & metric tracking, etc.).

Please indicate if you are applying to participate in this component: InsideTracker analysis of blood tests to optimize well-being and performance (this will involve uploading blood test results from your most recent physical or going to a local Quest Diagnostics for a blood test in the next two weeks, and they will send back a full report. DLP is covering the cost of the Essentials (showing 13 biomarkers) + Inner Age report. Should you want to add your own DNA results from Ancestry or 23andMe you can pay the $29 upgrade. If you'd like to upgrade to the Ultimate Package (43 biomarkers), you can elect to pay the $300 upgrade fee).  If this does not make sense yet, that's certainly understandable. All of this will be explained by the InsideTracker rep on our kick-off call.
