This form is intended to provide the school district with the numbers needed for planning purposes. This is NOT the declaration survey! That survey will be provided sometime next week (September 7-10).

Please understand that online learning this year will be a basic computerized curriculum for grades 6-12 and dedicated after hours class times for Kindergarten through 5th grade. These 6-12 computer-based courses may not necessarily align with the in-person instruction option. Therefore, families will be required to make the choice for either in-person or online learning for the entire semester (S1 & S2) for all grades, K-12. Additionally, choosing the online option may impact future course offerings when the student returns to in-person learning.
