Please note the following information for 2019:

Summer pick up will begin on June 4th and continue every Tuesday until school resumes.

This year we will not allow anyone to pick up for multiple households. Each family will need to pick up their own food.  You may authorize another person to pick up for your family. Please make sure to list them on this form.

If you miss your selected pick up time, the only alternate pick up will be on Tuesday evenings from 5-7pm at River Valley Food 4 Kids (407 North El Paso, Russellville)

Please review the guideline regarding income limit for 2018 before completing registration (you may be asked to provide proof of income):
