LIABILITY RELEASE
This is a legally binding Release made by me, to the University of Massachusetts (University). 

I fully recognize that there are dangers and risks to which I may be exposed by participating in during University of Massachusetts Science Olympiad. 

I understand that the University does not require me to participate in this activity, but I want to do so, despite the possible dangers and risks and despite this Release. 

I therefore agree, in consideration of and return for the services, facilities, and other assistance provided to me by the University in this activity, to RELEASE the University (and its Board of Trustees, officers, employees, and agents) from any and all liability, claims and actions that may arise from injury or harm to me, from my death or from damage to my property in connection with my participation in this activity. I understand that this RELEASE covers liability, claims and actions caused entirely or in part by any acts or failures to act of the University (or its Trustees, employees, or agents), including but not limited to negligence, mistake, or failure to supervise by the University. 

I recognize that this RELEASE means I am giving up, among other things, rights to sue the University, its Trustees, employees, and agents for injuries, damages, or losses I may incur. I also understand that this Release binds my heirs, executors, administrators, and assigns, as well as myself. 

I have read this entire Release, I fully understand it and I agree to be legally bound by it.
PHOTO RELEASE
In consideration of the opportunity extended to me to participate in the University of Massachusetts - Amherst Science Olympiad 2023 Invitational (the “Invitational”), conducted by the Trustees of the University of Massachusetts - Amherst (“UMASS”), on behalf of myself, or the minor child on whose behalf I am signing, I hereby grant to UMASS and the Invitational the right to use and to license others to use my or my child’s name, voice, actual and/or simulated likeness, caricature, photograph, any biographical material concerning me or my child, and my or my child’s participation and appearance in connection with the Invitational, for the purpose of UMASS's publicity, promotion, marketing, and fundraising and development efforts, including but not limited to the Invitational, with the understanding that such use may occur without limitation, in any and all languages, formats and media, now known or hereafter devised (including, without limitation, via the Internet).

I hereby grant to UMASS the right to photograph, film, or otherwise audio-visually record me or my child (including my or my child’s voice, conversation, and sounds), for the purpose of generating media to be used in connection with the rights and license granted above.

I understand and agree that my or my child’s appearance hereunder and/or in any media generated by
UMASS does not constitute employment and no payment will be due for services, and that the Invitational and the images, recordings, and media created in connection with it, as well as the copyright in the same, are and shall be owned exclusively by UMASS.

This Release shall be governed by Massachusetts law, and the parties agree to the exclusive jurisdiction of the courts situated in Massachusetts for any civil action arising under this Agreement.
SCIENCE OLYMPIAD PLEDGE
“I pledge to put forth my best effort in the Science Olympiad and to uphold the principles of honest competition. I have read and understand the National Science Olympiad Tournament Code Of Conduct and pledge (listed here: https://www.soinc.org/sites/default/files/uploaded_files/GeneralRules.pdf). I understand that failure to follow the Code of Conduct or rules established by the Tournament Director may result in penalty and/or disqualification.”
