The daily screening form is to help identify signs and symptoms that may put yourself or others at risk of contracting a communicable disease.  We ask that if you score less than 90%  you contact the activity supervisor prior to engaging in activities with the Rochester Avon Recreation Authority or Four80 Fitness.  If you score above 90% please present your results to a staff person on site prior to participation.
