Perry Elementary After School Program (Grades K-6)

Perry Elementary School is pursuing funding to be afforded the opportunity to start an after school program for the 2024-2025 school year. Your input in the survey below will help us in the grant application process. 

Our goal is to provide an after school program that consists of tutoring, counseling, physical, cultural, and STEM activities free of charge for our students. However, submitting a grant proposal does not mean that the potential Perry Elementary After School Program will be awarded.  

If you have a child who is currently enrolled in grades PreK-5th grade, please spend a few minutes to complete and submit this survey.
  • We are seeking feedback to see if this type of programming is needed in our community.
  • We hope to have at least 80% parent input in completion of this survey.
  • This program will be offered free for the students in K-6th grade for the 2024-2025 school year.
  • Only one survey per family is needed.
