2022 VBS Registration
2022 VBS: Vacation Bible School at Chisago Lakes Lutheran!!
Monday, June 27th – Thursday, June 30th

9am – 12 pm for Preschool Age:  4 year old (and toilet trained) thru 6th grade in the fall of 2022

ALL are Welcome for 9am-12pm VBS.  Afternoon Activities are currently FULL, and will open up, only if more space in vehicles becomes available!!!

Tuesday and Wednesday afternoon activities:  9am – 3pm for Kids WHO WILL BE ENTERING 1st thru 6th grade in the fall of 2022  A bag lunch is needed every day – dress for weather.

NOTICE OF FILMING/PHOTOGRAPHY:  CLLC will be using photos to help share, you irrevocable consent to and authorize CLLC to photograph and make sound recordings of your child and grant CLLC the right to use photographs and recordings for the purposes of CLLC.

By agreeing below, you understand that reasonable precautions will be taken to safeguard the health and well being of the participants in this VBS. My Child/ren has my permission to participate in CLLC’ VBS, June 27th – 30th, at Chisago Lake Lutheran Church.  I recognize that there are risks involved in participating in this activity and hereby assume all risk of injury, harm or damage to my minor child as they participate in this activity.  I hereby release and agree to hold harmless Chisago Lake Lutheran Church and its employees, organizers, and any volunteers assisting in the program, from any and all liability and claims arising out of my child’s participation in the programs and related activities. I hereby release Chisago Lake Lutheran Church and its staff and sponsors, from responsibility and liability for any injury or illness that my child may sustain during the event.  In case of emergency, where I cannot be reached, I hereby authorize Chisago Lake Lutheran Church to administer necessary first aid or seek emergency medical attention for my child/ren.  I hereby authorize an adult leader of this event, as agent for me, to consent to any x-ray examination; medical, dental or surgical diagnosis; treatment; and other hospital care advised and supervised by a licensed physician, surgeon, or dentist (as appropriate).  I understand that I am responsible for the health care decision of my minor child/ren and agree that my insurance plan is the primary plan to pay for the treatment that is given to my minor child.  Your signature also gives permission to transport your child/ren to all offsite afternoon activities.

Permission to share photos of your children on church's website and social media pages* Required.   By signing my name below, I consent to the use of my child’s photo, video, or likeness in promotional materials made by Chisago Lake Lutheran Church in Center City, MN, such as church website and other social media outlets. I also consent to allowing my child’s image to be recorded, either by photograph or video, and used during the VBS program or for future advertisement of Chisago Lake Lutheran Church, Center City, MN. Any other use will require my further consent. I understand that my child’s identifying information will not be provided unless I give permission at a later time.
