I recognize that my child(ren) will be participating in all or some of the following events, hereafter referred to as “the Activity": Wednesday evening meetings, and activities associated with these meetings, summer book studies, and other events and activities.

I certify that I am cognizant of the inherent dangers associated with participating in the Activity and with the fact that participating in the Activity may take place outside of, or off premises of The Trails Church.

I understand and agree that neither The Trails Church, nor its trustees, representatives, instructors, or agents may be held liable in any way for any occurrence in connection with the Activity which may result in injury, harm, or other damages to my children or my family.

As part of the consideration for being allowed to enroll and participate in the Activity, I hereby personally assume all risks in connection with participation in the Activity. I further release The Trails Church, its trustees, instructors, agents, and representatives for any injury or damage which may befall while enrolled in or participating in the Activity. I further agree to save and hold harmless The Trails Church, its trustees, agents, and representatives from any claim by me, or my family, estate, heirs or assigns arising out of enrollment and participation in the Activity. I also authorize The Trails Church to render or obtain such emergency medical care or treatment as may be necessary should any injury, harm or accident occur while participating in the Activity. Additionally, I understand and agree to be transported by a member of The Trails Church in good standing, to and from the Activity, in their own personal vehicle, or a church-rented vehicle.

I further state that I am of lawful age and legally competent to sign this affirmation and release: that I understand the terms herein are contractual and not a mere recital: and that I have signed this document of my own free act and volition. I further state and acknowledge that I have fully informed myself of the contents of this affirmation and release by reading it before I have signed it.

I hereby grant permission to The Trails Church (hereinafter called the “Church”) to use said Minor(s)’s photograph, likeness, image, voice, or performance on its internet website, video or film clips, or other official Church publications at the sole discretion of the Church and to be used in whole or in part of any and all broadcasting, audio/visual, and/or exhibition purposes in any manner or media, in perpetuity, throughout the world.

I represent that I have the right, capacity, and authority to enter into this agreement (hereinafter called “Agreement”) and that the Minor’s appearance and the rights I have granted in this Agreement will not conflict with or violate any commitment or understanding I have with any other person or entity.

I understand that once the Minor(s)’s image appears on the Church’s website, the image can be downloaded by any computer user. I expressly release the Church, its officers, directors, employees, volunteers, and agents from any and all claims arising out of the use of the Minor(s)’s photograph or image on the Church’s website. I agree to indemnify and hold harmless the Church, its officers, directors, employees, volunteers, and agents from and against all claims, losses, expenses, and liabilities of every kind including reasonable attorney’s fees arising out of the inaccuracy or breach of any provision of this Agreement.

In consideration for said Minor(s)’s appearance and/or performance on the Church’s internet website, video or film clips, or other official Church publications and all rights granted hereunder, I do not require or expect any monetary or financial payment or incentive or benefit but consider the Minor’s appearance and/or performance to be a form of service to the Church.

This Agreement represents the entire understanding of the parties and may not be amended unless mutually agreed to by both parties in writing.

I represent that I am the parent and/or legal guardian of the above-named Minor(s). I agree that we both shall be bound by this Agreement.
