Instructions

1.      The Island ECC church office requires a photocopy of your passport and Hong Kong Identification Card and an emergency contact. These must be on file before you will be cleared to travel with the team.

2.      Submission of this form does not mean the application is approved. The applicant will be informed of the outcome by trip leaders once all applications have been reviewed.

3.      This form will be held in strict confidentiality by Island ECC Outreach staff and mission team leaders.

4.      ALL questions are mandatory; incomplete forms will not be accepted.

5.      This application consists of 6 sections and can take up to 30 minutes to complete. All applicants, including returnees, will need to complete this form.
