1.樂暢空間工作室（本工作室)取得您的個人資料，目的在於進行本工作室之相關工作（如貴賓資料建檔、活動通知、遺失物聯繫、網路宣傳等）。蒐集、處理及使用您的個人資料將受到個人資料保護法及相關法令之規範。
2. 本中心蒐集與使用您的個人資料，包含姓名、性別、身分證字號(或居留證號)、生日、聯絡方式(電話、E-mail、地址、服務單位相關資訊)等等。
3. 您同意本工作室因職務所需，處理及使用您的個人資料。本中心利用您的個人資料期間為即日起永久保存，利用地區主要為臺灣地區，部份個人資料將因應使用目的可能擴至其他地區。
4. 您可依個人資料保護法，就您的個人資料向本工作室：(1)請求查詢或閱覽、(2)製給複製本、(3)請求補充或更正、(4)請求停止蒐集、處理及利用或(5)請求刪除。
5. 本同意書如有未盡事宜，依個人資料保護法或其他相關法規之規定辦理。
6. 您瞭解此一同意書符合個人資料保護法及相關法規之要求，具有書面同意本中心蒐集、處理及使用您的個人資料之效果。

一、為符合我國個人資料保護法、著作權法與民法肖像權等相關規定，本工作室參與之活動照片及影片均須取得授權(因幼兒未成年，故須取得法定代理人同意)。
二、關於本工作室活動照片及影片之運用，其形式為：
(一) 將幼兒活動照片張貼於本工作室網站供家長們瞭解幼兒活動情形及交流。 (二) 將幼兒活動照片及影片做為課程記錄、發表、宣傳、比賽或著作出版等使用，幼兒的個人資料也將受到匿名保密。
(三) 幼兒活動照片及影片之運用僅供本工作室紀錄活動及宣傳使用，不做其他商業用途。
三、若有造成家長或幼兒困擾時，本工作室仍可接受協商，取消幼兒之肖像使用權。

本人(以下簡稱甲方)同意授權樂暢空間工作室(簡稱乙方)取得並使用本人及子女肖像、姓名、聲音（包括活動照片及影片），出現在乙方的網站、粉絲專頁、官方LINE群組頁面中，同時，本人及子女的個人資料也將受到匿名保密，僅供乙方課程記錄、發表、宣傳、比賽或著作出版等使用。簽屬本同意書，表示雙方願意接受下列所有條款與規範：一、甲方同意授權由乙方使用其肖像（包含照片及動態影像）、姓名、聲音，授權乙方從事以下行為：(一)、乙方得以各種管道或印刷方式呈現授權內容之全部或部分並可公開發表，及著作法賦予著作人所擁有之權益，且可無須再通知或經由甲方同意。(二)、雙方同意單獨使用授權肖像來展示及宣傳。(三)、如乙方所提供之創作備份於甲方，甲方使用時也應尊重乙方創作權，公開發表時須註明原創者資料，不得侵犯智慧財產權(例如:讓觀賞者誤以為是他人作品)。二、乙方需保密甲方非個人宣傳之私密資料(例如:電話、地址、身分證字號等)，未經甲方同意不得擅自外流。 I (hereinafter referred to as Party A) agree to authorize Le Second Floor Kids Studio (hereinafter referred to as Party B) to obtain and use my and children’s portraits, names, voices (including event photos and videos), appear on Party B’s website, fan page, official LINE group Account, also, the personal information of myself and children will also be anonymous and confidential, only for the use of Party B's course records, publications, publicity, competitions or book publications.
