Custom Embroidered Beanie Extras Form
Thank you for participating in the extras custom embroidered beanie stocking!  After purchase, please fill out the form separately for each beanie that you purchased.  The color/style/size of beanie you select on the form MUST match the color/style/size on your order!  You get to chose your embroidery and embroidery colors. Please take your time and double check your selections as once submitted, your form is final.  Please fill out all forms by Monday morning 9am CT.
Embroidery placement!  Embroidery will be placed to the side of the beanie on all options EXCEPT for the double pom beanies and child ear beanies, which will be placed in the middle.
Embroidery disclaimer: Minor imperfections in embroidery stitching are very normal.  You may see some stitching fall outside of the border, slight gaps between border and stitching or stitches that are not completely smooth.  There may be some puckering around the embroidery where the material doesn't lay flat when off the head, but is not perceivable while being worn.
All beanies should be hand washed and laid flat to dry.

See below for thread color options.  You may suggest specific colors or give us your general vision, 'Traditional colors',  'Neon funky GN' or 'Pastel Girly with some variegated', for example.  Swim Shady, Alien, Monster Truck and New Butterfly have 'blends'.  The rest do not.  Reminder that these are suggestions and although we do our best, we cannot always capture your vision.  Unfortunately, we cannot offer a remake for a missed vision or misinterpretation of your suggestion.
