Please write their name below exactly as you would like it to appear on an award they will receive at the end of the year (i.e. nickname vs. formal name, middle initial).  If it is not your child's anniversary year, please skip this section.

Your answers to this form will be used to create your Quickbooks billing from Sept-May this year. Tuition is spread evenly over each month regardless of how many weeks/classes are in any given month. The billing from September - May will come once a month by the first of the month through an e-mail invoice from Quickbooks. A late fee of $25 will be applied to any accounts more than 10 days past due. Payments can be made in check, cash, or credit card. You may elect to enroll in autopay if you choose and can set that up through your first invoice when it arrives.

Pay in full discount for team tuition by September 12 – 5% off tuition
