I agree to release the Oak Lawn Tennis Association, its directors, agents, and designees (collectively “OLTA”) from any and all liability relating to injuries that may occur during any OLTA-related activity, either on location or during transportation to/from any related event. By accepting this agreement, I agree to hold OLTA entirely free from any liability, including financial responsibility for injuries incurred, bodily/emotional injury, damage to reputation, or any other harms, regardless of whether injuries are caused by negligence.I also acknowledge the risks involved in playing tennis. These include but are not limited to physical and emotional injury. I agree that I am participating voluntarily, and that all risks have been made clear to me. I will also make every effort to obey safety precautions and will ask for clarification when needed.

Image/Photography waiver

Photographs, video or other types of media may be taken or used. I grant OLTA permission to capture my image and likeness in photographs, video, recordings, or any other media (collectively “Images”). I acknowledge that OTLA will own such Images and grant permission to copyright, display, publish, distribute, use, modify, print and reprint such Images in any manner whatsoever related to OLTA business, including without limitation, publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I waive any right to inspect or approve the use of the Image by OLTA prior to its use. I release and hold OLTA harmless from any and all liability arising out of the use of the Images in any manner or media whatsoever, and waive any and all claims and causes of action relating to use of the Images, including without limitation, claims for invasion of privacy rights or publicity. By agreeing to these terms, I forfeit all right to bring legal action against OLTA for any reason.
