Completion of the Incident Report is mandatory for ALL incidents involving injuries regardless of whether medical attention was required. Report is to be completed within 48 Hours. 


One copy of the incident form should be completed by the Coach or Manager of the team, and one should be completed by the parent or guardian if they were in attendance.
