2021 Women's Open Lake Tahoe Tournament Registration Form
Each team should designate a TEAM CAPTAIN & a TEAM POINT OF CONTACT. All communication will be sent to those 2 people. Individual players will register at www.truckeetribe.club  once their team captain notifies them that the team is registered. This FORM is ONLY for TEAM REGISTRATION.

Deadlines:
July 1st- team registration and full balance due ($1200)

July 10th: date to cancel the tournament if there aren't enough teams or the State of CA changes it COVID mandates regarding adult recreational sports.

July 10th: Team rosters with players name and email are due to wlaxtahoetournament@gmail.com.

July 27th: Player registration is due. Players who do not complete registration and waivers on www.truckeetribe.club will not be permitted to participate in the tournament.

I understand that all jerseys/t-shirts worn by our team must have numbers on the front and back and if reversible on the front & back of the other side. There can be no duplicate numbers.   Rule book penalty is loss of possession at the start of each half and any overtime periods.

I understand that the tournament takes place on July 30, 31 and August 1st and that our team must be available for games on each of those days.
I understand that the tournament requires that ALL players have a current and valid US Lacrosse ATHLETE/PLAYER membership in order to be covered by event insurance. (must be valid until Aug 2, 2021)
I will submit our team roster by July 10th with player name, and email address to wlaxtahoetournament@gmail.com so that the tournament organizer can send my team the link to complete the player registration, agreements and COVID waiver. (Type full name below to indicate your agreement)
I understand that all teams are limited to 18 players on their roster.                                              *If our team wishes to petition for a waiver to have more than 18 we will provide a request via email to wlaxtahoetournament@gmail.com by July 10th in order for the tournament advisory committee to review and provide a ruling. No reasonable request will be denied.
DUE TO COVID we can not allow players to be shared between teams.-----I understand that if our team is found to have more than 18 or to have borrowed players during a game that the game will count as a win for the non-offending team and the venue manager will be notified. The tournament is not responsible for the consequences imposed by the venue for violation of the COVID guidelines.
The tournament registration fee is $1,200.00 and due in full by July 1, 2021. I understand that if we do not pay in full by that date we may loose our spot in the tournament. *
