To book, please complete this form & make payment via bank transfer - your space/my services will not be secured until at least a deposit has been paid. 
A non-refundable deposit of £10 will secure your space - or you can pay the full amount (£65)
The remainder of the fee is due to be paid at least 10days prior to the course start date, if not before.

Booking Terms & Conditions: By completing this form you are committing to paying for & undertaking classes or a course with me, Sally Carter (DoulaSlingSally).  

A non-refundable deposit of at least £10 must be paid upon booking & the remainder of the fee is due at least 10 days prior to the course start date. (26/03) 
You may cancel your booking at any time. 
If there is less than 10days notice before the start of the course, or you cancel part way through the course, no refund will be given. (Unless under exceptional circumstances - at my discretion) 

A minimum no. of bookings are required to run an in person course, I reserve the right to cancel if the minimum no. of bookings is not reached, in which case an alternative or full refund will be offered. 

As a birth doula, there may be need to reschedule a session if I am attending a birth; I will endeavour to give as much notice as possible of the postponement & will offer an alternative/catch up session - no refund will be issued. 
 
Any personal data shared here will be kept safe under GDPR rules & not shared with or passed onto any other parties.
