This form is required to obtain Loyalty Points. On the back of every booster pack wrapper, there is a resource symbol with the word "Redemption" printed above it. When you are mailing in your packs for redemption, you must also print and mail this form (A copy will be emailed to you) in order for your Loyalty Points to be processed correctly.
