Please complete this form below to request one (1) mask from the USC Mask Makers. You will need to complete this form multiple times if you are requesting more than 1 mask. This form is for attendees of USC Westlake, and not for public use or distribution.
Since this is a volunteer initiative, we cannot guarantee the type of mask you will receive. Our sewers are creating masks that they are comfortable making (i.e. tie back, elastic, filter pocket, pleated, etc.), but all will cover the nose and support under the chin. If you have to have a specific type of mask, please do not complete this form. We are not able to take custom orders at this time.
Masks will be dropped off on your porch or in your mailbox. If you live in a secured area (gated community/apartment), please provide detailed instructions below to guarantee a safe, unattended delivery (meaning that masks may be delivered anytime, day or night) of your mask.
